The focus is on premature puberty, which these days usually means
virilization as a result of what is known medically as premature
adrenarche, or pronounced adrenarche, or exaggerated adrenarche. The
symptoms include pubic hair and body odor.
In girls, this virilization is often the first symptom of polycystic ovary syndrome (PCOS).
Early virilization in boys is less apt to be considered pathological, and has received less attention. But as happened with girls, this attitude will likely change as the long-term effects become more apparent.
High insulin levels are known to cause an exaggerated adrenal response to ACTH. Insulin insensitivity is a characteristic of these virilized children, and the insulin sensitizer metformin reduces their androgen levels.
